Description
Aims:
To provide an additional opportunity to:
· support the process of transition from primary to secondary for the most vulnerable pupils with high needs
· to identify any background characteristics of pupils and families that may hinder the successful and positive transition for each pupil
· network between Y6 and Y7 colleagues involved in transition
Please ensure that you have completed relevant transition forms for secondary colleagues to take back to their settings. Copies of all forms will be emailed to school SENCOs.
